Description	Radiopaedia case ID: 73730 Image ID: 51955732 Image stack position: 141/355 Plane projection: Axial Study description: FDG-PET/CT Study findings: Bilateral large FDG avid adrenal lesions with numerous surrounding malignant FDG avid lymph nodes near the upper abdominal aorta and IVC. No evidence of metastatic disease elsewhere. The most likely etiology was considered bilateral adrenal metastases with metastatic lymph nodes in the upper abdomen, but a primary malignancy was not identified. Synchronous primary adrenal carcinomas with adjacent metastatic nodes was considered possible. Modality: Nuclear medicine System: Oncology Findings: Large bilateral soft-tissue density adrenal lesions. No internal fat or calcification. LEFT ADRENAL LESIONPre-contrast: 33 HUPortal venous contrast: 59 HU15 minute delayed: 58 HUAbsolute washout: 3.8%Relative washout: 1.7% RIGHT ADRENAL LESIONPre-contrast: 28 HUPortal venous contrast: 33 HU15 minute delayed: 30 HUAbsolute washout: 60%Relative washout: 9.1% The washout characteristics were indeterminate. Size > 4cm is concerning for malignancy, including metastases or bilateral adrenal cortical carcinomas.
