  • Study findings: Anteroposterior chest radiograph shows double density of right hemidiaphragm. A rounded curvilinear structure (white arrow) is projected above the true right hemidiaphragm (black arrow). This rounded curvilinear structure appears to be of fat density. Lateral chest radiograph shows three curvilinear density. Blue arrow points to the right hemidiaphragm, red arrow point to the left hemidiaphragm and black arrow points to a third curvilinear structure projected above and silhouetting the posterior right hemidiaphragm .

  • Findings: CT thorax coronal view demonstrates a right Bochdalek hernia.  There is herniation of peritoneal fat through a congenital diaphragmatic defect, Bochdalek foramen, in the posterolateral right hemidiaphragm. Bochdalek hernia is usually left sided and maybe an incidental finding in as high as 10% of asymptomatic adult.1 As demonstrated in this case, the herniation content is usually retroperitoneal fat and less often kidney (right sided).
