Summary:

Description
  • Radiopaedia case ID: 53805
  • Image ID: 30738582
  • Image stack position: 18/150
  • Plane projection: Coronal
  • Aux modality: C+ delayed
  • Modality: CT
  • System: Head & Neck
  • Findings: Heterogeneously enhancing right maxillary sinus mass with significant surrounding structural involvement and severe bone destruction. Superomedially mass extends into ethmoid air cells by eroding perpendicular plate of ethmoid, nasal cavity involving nasal turbinates by destroying nasal surface, as well as palatine and alveolar processes of maxilla. Mass further extends medially into parapharyngeal space, mucosal pharyngeal space, pterygopalatine fossa, and retromaxillozygomatic space, causing severe erosion of medial and lateral pterygoid plates with loss of intervening fat planes between medial and lateral pterygoid muscles and temporalis muscle. There is marginal erosion of greater wing of sphenoid and orbital surface of maxilla superiorly. Laterally, mass extends into masticator space by eroding zygomatic process of maxilla and temporal process of zygomatic bone, as well as eroding inner aspect of ramus of mandible. No extension across the midline is appreciated. Posterior right upper dentition is floating.
