Summary:

Description
  • Radiopaedia case ID: 90044
  • Study ID: 107205
  • Image ID: 55130534
  • Plane projection: Axial
  • Modality: CT
  • System: Head & Neck
  • Findings: There is a right nasoorbitoethmoid (NOE) fracture with intact large central fragment to which the medial canthal tendon is expected to attach. Fractures extend into the orbit, involving the right medial orbital wall and orbital floor. The depressed area of orbital floor measures 16 x 12 mm (AP x TV), herniated 5 mm into the maxillary sinus (approximate volume based on ellipsoid is 0.5 cc). The medial orbital wall (lamina papyracea) fracture is minimally displaced. There is small volume intraorbital emphysema, but no retrobulbar hematoma. The globe appears intact, within the limits of CT. There are multiple nasoseptal fractures, involving the nasal bones, frontal processes of the maxillae, anterior nasal spine, and bony septum. These are associated with leftward deviation of the nasal dorsum and accordion configuration of the bony septum. There is hemorrhage in the right maxillary sinus.
