Exploration through a posterior sagittal approach demonstrated a silvery white epidermoid tumor (white arrow) occupying the presacral space. The tumor was poorly circumscribed and firmly adhered to surrounding tissues containing bilateral S3 sacral nerve roots (white arrow heads) and dura (asterisk).
