English: f0015: Axial (left), and sagittal (right) MR images of a 40-year-old patient with recurrent motor seizures (epilepsia partialis continua) of the left hand since the age of two, due to cortical dysplasia in the hand area of the right motor cortex. The MEG motor findings suggested functional reorganization with lateralization of the left hand to the left hemisphere (task-related power decreases in the beta band during self-paced movements of the left hand (red) and right hand (green). Identification of the epileptogenic focus was necessary with invasive techniques. The MEG results supported additional functional mapping as well and therefore, subdural grid monitoring of the right hemisphere was performed, showing an extensive and scattered area for motor hand function. Surgery was performed with premotor removal of tissue and multiple subpial transections of the right hand motor cortex. Postoperatively, the patient had no neurological deficits of the left arm and a significant seizure reduction. L: left; R: right; A: anterior; P: posterior. |
